The Manager of Care Management supervises a team (including RNs, Social Workers, and Coordinators) to coordinate services and ensure program mission alignment. Responsibilities include setting team goals, providing feedback, coaching, and conducting performance reviews. This role requires collaboration across Clinical Operations, analyzing metric data, and identifying improvement opportunities.
Duties/Responsibilities
- Develop and monitor team goals; provide feedback and coaching; conduct performance reviews.
- Recommend staffing levels for effective care management.
- Oversee delivery of care plans and authorized services.
- Collaborate with Clinical Operations' core areas.
- Analyze reviews, metric data, and PHI audits for improvement.
Minimum Qualifications
- Associate's degree
- NYS RN, LSW or relevant clinical license
Preferred Qualifications
- CCM certification
- Supervisory or training experience
- Clinical work experience
- Knowledge of member satisfaction/incident management and regulations
- Knowledge of quality improvement methodologies
Compliance & Regulatory Responsibilities
Ensure team compliance with department and regulatory requirements. Supervise and audit compliance, including HIPAA standards and electronic PHI storage systems.
Hiring Range
$100,900 - $145,775 (Greater New York City Area) $86,500 - $128,690 (Other Locations)
