Responsibilities
The MDS Nurse oversees the Resident Assessment Instrument (RAI) process, conducting assessments and coordinating care plans for assigned residents. Key responsibilities include:
- Completing MDS, CAA’s, and care plans within regulatory timeframes.
- Conducting comprehensive resident assessments through physical evaluations, interviews, and chart reviews.
- Collaborating with interdisciplinary teams to address resident care needs.
- Integrating information from hospitals, consultations, and external agencies into assessments and care plans.
- Initiating significant change MDS assessments and facilitating PPS meetings as needed.
- Staying updated with AANAC requirements and PPS guidelines.
Qualifications
- Education: Registered Nurse (RN) with active licensure.
- Certification: AANAC certification preferred; RAC-CT a plus.
- Experience: Previous experience as an MDS Nurse preferred, with knowledge of the RAI process and PPS principles.
