The Support Coordinator plays a crucial role in providing non-clinical assistance to care and case managers. This position involves creating cases, managing events, conducting telephonic outreach, and coordinating various tasks related to member care.
These tasks include communication with members, providers, and community-based organizations, along with handling mailings, faxing clinical requests, and forwarding Individual Health Care Plans. The Support Coordinator works within a designated clinical team such as Care Management, Utilization Management, or Behavioral Health—and may be responsible for additional departmental duties as needed.
Key Responsibilities
- Conduct telephonic outreach to members, assisting with benefit inquiries, scheduling appointments, and conducting initial assessments to identify potential health risks requiring further intervention.
- Serve as a liaison with healthcare providers, handling authorization requests, referrals, scheduling of tests or visits, and faxing of care plans.
- Ensure accurate and timely documentation of all member-related information in compliance with internal policies.
- Efficiently manage a large caseload, addressing member needs while optimizing quality of care and cost outcomes.
- Act as a point of contact for screening and identifying risk factors for members and escalating calls to higher-level care when necessary.
- Ensure timely communication of member concerns to care managers to support ongoing care management.
- Assist with any additional tasks as assigned.
Qualifications
Minimum Requirements
- High School Diploma or GED equivalent.
Preferred Qualifications
- Proficiency with basic Microsoft Word and Excel.
- Multilingual skills preferred—Spanish, Russian, French, Creole, Mandarin, Cantonese.
- Experience in managed care, particularly in a Call Center or Care/Case Management Department.
- Familiarity with documenting calls and member interactions in computer systems.
- Knowledge of medical terminology.

Compensation and Benefits
Hiring Range
- Greater New York City Area (NY, NJ, CT): $39,208 - $52,000
- All Other Approved Locations: $34,091 - $49,920
The final salary offer will be determined based on the candidate’s experience, education, and qualifications, along with other relevant factors.
